Affordable care act and Supreme court influence on legislation.
Trump: Claims its socialist policy, denies amount of americans with
pre-existing conditions.
Biden: Claims private insurance will remain, Obamacare will be expanded without forced switching.
Trump:Socialist healthcare(repeated claims)
Biden:pre-existing conditions are bad, we're going to remove them.
Also links COVID patients to pre-existing conditions.
Trump: VA leads to 308k veterans dying, blames Biden for it.

NYTimes highlights that Trump doesn't condemn Proud Boys(in fact asking them to 'Stand by') and being a bully in the debate.

Overall Biden was not exhibiting dementia as right-wing hoped him to do, his responses while muted were sober.
Considering that Trump attacks on Biden were not really relevant to debate and Biden dodged many questions, the debate has no clear winner:
It can be considered Trump 'dominated' the debate, but no clear arguments were won by him. I think Wallace-Trump interaction was the key dynamic in the debate and Wallace didn't manage to control it.
Trump was adept at redirecting attention towards his pet issues and
debate largely revolved about them, without substantial progress on
actually 'debating' anything - its as if both sides got free advertising slots on TV to proclaim some positions from their platform.
The only thing they're 'debated' is interpersonal questions about the Trump/Biden character traits and not actual policy values.
I rate this debate 3/10.

Fact checks are mostly ok, except the FBI statement that antifa is an ideology not a group. Its technically an organization that exhibits the anti-fascism ideology, like a political party.
NYtimes is pro-Biden in his treatment of fact checks, but it feels that Trump answers fact checks are left in intermediate state of "true, but overblown", which indicates that NyTimes has little arguments against Trump or not willing to analyze the issues in detail, conceding the point.
Overall Trump loses on actual issues like environment/healthcare and gains on election fraud, protests and general political issues that appeal to right-wing.
An unbiased rational observer would declare Biden the winner simply by
discarding all untrue/biased statements by Trump(Populist), but he would appear too weak and compromising to appeal to anyone(centrist).
Its a debate between a Centrist backed by media with a Populist backed by conspiracy theorists and right-wing culture.

Biden has to appeal to left-wing, he probably lives in the mental 90's
where Democrats were centrist. Democrats are now undeniably left-wing. If Biden tries to "win both sides", it might alienate progressives and minorities(Latino) who want firm left-wing policies and guaranteed welfare/safety nets.
Trump has trouble with actual policies/plans and relies on bravado/self-advertising too much(as if His administration is some Trump Branded business venture that he needs to promote each minute).

Hot take:
It wasn't actually a debate, its psycho-poetic advertisement for white supremacy by a deranged boomer feeling the end of his career.
I think "Stand By" will be used as some new meme, its incredibly clear 'dog-whistle' to anyone (not just twitter SJWs) that Trump has lost mainstream appeal and will support an extremist paramilitary just to 'own the libs'. Elections aren't real in his mind, so Biden can't actually 'win', and it legitimizes powers behind Trump that are not backed by 'law&order'. Its a transition from 'law&order' to some political coup or civil conflict. I don't know how they will de-escalate this.

Its likely to be some sort of internal coup and free reign for paramilitaries like Latin American countries when republics collapse to dictatorships. The opposition will be able to gather lots of support from mainstream and elite - a crackdown from the top wouldn't work.
If Trump denies election results(Biden leads in the polls),
the refusal to transition will be justified by his political cliques and paramilitary groups, but as they contest the law they discredit the
concept of free elections and cement the idea of 'political strength' as sole determination of valid leadership (aka "Strong Leader we need" mentality), which plays right into a scenario of patriotic revival,
interpreted as white supremacist coup and fascism by the left:
the confrontation will lead to open civil conflict on the contested
areas where Biden and Trump claim political power:
A situation of "Dual powers" that is a direct precursor to civil war,
where local areas pledge their allegiance to specific party and
refuse authority of unified republic. Its clear this is powderkeg
of political anger and mutual hate brewing when "authority" is shifted
towards "moral power" from legislative and bureaucratic systems,
therefore enabling charismatic leadership and popular movements to
assert direct control over political process - gaining territory
and influence on specific localities(like e.g. CHAZ autonomous zone and various religious compounds) where their power is not legalized
but accepted as true authority by other means(religion, ideology,etc).
